Point-of-Care (POC) Testing: POC testing conducted to assess HbA1c and lipid panel, offering insights into metabolic health.

Vitals: Height, weight, and blood pressure measured.

Toe Brachial Index (TBI): Toe pressure assessed to screen for peripheral artery disease, a condition often linked to cardiovascular health complications and amputations.

Education: Personalized health education offered based on patient's assessment results. Topics covered include diabetes, high cholesterol, high blood pressure, and peripheral artery disease, as well as, guidance on health insurance and primary care physicians.
